Hello doctor,

My wife had an itching and rashes problem around private parts, including the lower side of the stomach and thigh area. Initially, it was a small patch; but it spread slowly. The patches spread to all areas below her navel; now, there are big patches below the navel and thigh area. We visited a doctor who gave certain medicines, lotion, and powder, but it did not help. I am sharing the list of medications with you. I used Lulibor 1% ointment (Luliconazole), Abzorb Clotrimazole powder, Capsule Itraver 200 mg (Itraconazole), Tablet AF-150 (Fluconazole) and Tablet Atarax 10 mg (Hydroxyzine). It is spreading slowly to all areas below the stomach and thigh. She is also getting some rashes on her face on the side of her lips. Please suggest some good medicine.

As per the history you have provided and the images shared (attachment removed to protect the patient's identity), it is a fungal infection called Tinea. Tinea spreads fast and is contagious. It is not a life-threatening problem. It is a skin infection that completely goes away. When appearing on the body, including the abdomen, tinea infections are called tinea corporis. Tinea faciei is when it appears on the face, which is your wife's case is on the edge of her lips; when under the abdomen and in the groin area is called tinea cruris. It starts after being infected by another infected person due to excessive sweating and immune deficiency. Kindly follow the following medication. Ketoconazole soap to be used twice daily. After lathering with water, it has to be used and applied only on the affected parts and not on normal skin. Keep the lather on the affected area for five minutes and wash it off. On the rest of the areas, an ordinary soap can be used. Terbinafine cream to be used twice daily on the affected areas alone. After taking a bath with the soap, towel dry the areas well. There should not be any moisture, and then apply the cream. Ensure to wash your fingers well with soap after applying the medication on the affected area, or you may also use gloves to apply and make sure the cream is well absorbed. Terbinafine dusting powder - It has to be used in the afternoon and evening. Dust the powder on the affected area, especially between legs and under the abdomen where there are folds. Moisture should not be kept stagnant in those areas. Take Tablet Terbinafine 250 mg once daily to be taken after food for six weeks, preferably at night. Take Capsule Becosules Z (Vitamin B complex, Vitamin C and Zinc) to be taken once daily after food in the morning for two months. Take Tablet Cetirizine 10 mg to be taken one hour before sleeping for three weeks. It is for reducing the itching. Make sure she does not go out or drive after taking the tablet, as some may feel sleepy after taking the tablet. Sweating has to be avoided. Whenever she sweats, she needs to dab the area well with a towel or tissue and keep her body dry. Wear loose clothing and air dry under a fan for a few minutes each day to let the moisture dry up. Make sure nobody at home has similar lesions. If yes, they need to be treated as well. Also, make sure not to share any of the belongings, including towels and combs or dresses, as it may spread from person to person. Even a small lesion takes almost a month to go away and heal completely. Since the lesions are widespread, it would take a few months to go off completely. Therefore give it some time and be patient as it may take some time to heal. Do not worry. It will heal slowly and effectively with proper treatment. Do follow up after four weeks to update me regarding her progress. Stay healthy, happy, and safe. Get well soon.
