Hi doctor,

I have Hashimoto's, taking Levo. My hair started to receding with that sickness. Sometimes it just came back fully then receding as a male pattern. I am a woman. My hair came back four times in eight years by taking Iron or add extra Levo. but it is not staying and started receding again. What can I do to stop it? Previously after seeing my picture doctor told me I have hormonal alopecia. I also tried taking Finasteride for five and a half months, but no improvement. My testosterone, DHT, DHEA all are at the low bottom of the normal level. Kindly help me.

With reference to your photo and medical history (attachments removed to protect the patient's identity), I suggest the application of Minoxidil 10% solution to the scalp daily would be of great help to you, but it needs to be applied for several months. Initially, there may be an increase in hair fall, which would stop in four weeks or so. There are procedures also which can be done by dermatologists like PRP (platelet-rich-plasma), Mesotherapy, etc. Also, get Vitamin D, Vitamin B12, hemoglobin, and serum ferritin levels checked. Thank you. Do you think whether DHT is taking my hair or any other hormone? Minoxidil 5% did not help on trying it for 6 months. We do not have 10% over the counter. Could you prescribe it? All of those blood tests are done already. They are in range as I take vitamin D, B12, and iron supplements. Ketoconazole 2% shampoo, which you told was good. Could you also prescribe it as it is not over the counter? I tried PRP and did not help at all. Do you have any idea why these treatments fail even after taking Finasteride? I want to know whether it is a DHT problem. Please advice.

See Hashimoto's seems the main cause in your case. You have not sent your blood reports. Minoxidil should be applied for many months. Also, PRP (platelet-rich plasma) should be taken for 6 to 10 sessions. In case, if you do not have dandruff, do not apply Ketoconazole shampoo as it may aggravate the hair fall. Regards.
