Hi doctor,

I am a travel nurse and I move constantly. I had ten different prescription strengths as my doses are changed often. After that, I got wrong dosage of medicines. It was an error done before few months. My prescribed dose is Synthroid 125 mcg and Cytomel 25. As my doctor was adorable, she told me to take Cytomel 50 mcg. I was running throughout the day due to my new job and I did not pay attention to anything. Accidentally, I took 187 mcg and 50 mcg for weeks. After that, my hair started to shed a lot and I was hot and irritable. It took weeks to realize the mistake done by me. When I became symptomatic, I consulted my doctor. The doctor told me to take Synthroid 137 mcg and Cytomel 25 mcg. After that, I was fine without hot, irritable and I slept better for the last two days.

On my blood work, I had a TSH of 0.06 mIU/mL, FT4 of 1.5 ng/dL, FT3 of 3.2 pg/mL, and iron of 54 mcg/dL. I became terrified to take my medicines. My hair came in my hands. Do I need to skip Synthroid for few days? I think that even taking it as a low dose increases the problem. I am not worried about Cytomel as the level is in the midline and it has a shorter half-life. I feel as if I have taken my grandparent's blood pressure medicines by mistake. Can I have Synthroid pool? Will the lower dose continues to suppress my TSH and elevates the FT4 levels? Please explain me in detail.

My hair is coming out a lot. I have normal levels of vitamin D and B12. I take a massive protein diet with low carbs. When I had an iron level of 17 mcg/dL and a TSH level of 4.62 mIU/mL. I had a full-blown telogen effluvium. I had shedding of hair for six months and I have lost fifty percent of my hair. Is this happening again? After few months, my hair had some growth. Can my TSH return back to normal after few days? Is it possible for me to have a decrease in the level of FT4 after few days? I am terrified to wash my hair and to take medicines at this point.

You should omit to take both the medicines for a day or two days. Start taking Cytomel (Liothyronine) 25 mcg and Synthroid (Levothyroxine) 137 mcg. If you have any symptoms, then decrease the dose of Cytomel. You can take Biotin 10 mg once a day, Zinc 22 mg once a day for three weeks, and Magnesium 250 mg for a week. Eat healthy foods. Try to manage anxiety and stress levels.

Please help me to understand the mechanism. I have skipped the doses for three days. Tomorrow morning, I am going to start taking 137 mcg and 25 mcg. Can I have the symptoms due to intake of Cytomel and Synthroid? FT4 levels are higher and they are closer to the higher side of the scale. How long will it take to return back to normal? Is it good to take Cytomel 50 mcg or 25 mcg? Is it an overdose? I have poor hair growth. Is this telogen effluvium coming back again for the next six months? Do I need to take iron supplements? Last year, I took a higher amount of it and the doctor told me to stop taking it as it was more than 100 mcg/dL.

Within six months, it came back to 54 mcg/dL. Do I need to take it daily or weekly? We do not burn iron and overload is dangerous. I am taking biotin, zinc, vitamin D, magnesium, and selenium. I am taking low carbs, with lots of fresh vegetables and one citrus fruit daily. I am taking three liters of water daily with moderate caffeine. My heart rate seems to be around 109 beats per minute. While waking up, I have a heart rate of 64 beats per minute, and resting heart rate is 78 beats per minute and while moving, it is 90 beats per minute.

I had chest pain before a couple of weeks two times. After that, I realized that I am taking heavy foods. I suffered from this problem for four to five weeks. As it was twelve degrees here, I was unable to notice heat intolerance here. I waked up during sleep. I had a couple of bouts of nausea after taking caffeine. I was not able to do my workouts and daily activities. After that, I felt that I am a normal human being. Now I am concerned about my hair. My scalp and hair are sensitive to thyroid hormones. Kindly suggest.

Skipping the dose for three days helps to resume the new dose as the medicine form of thyroid replacement gets cleared from the body within a day or two. You need to reduce the dose of Cytomel as T3 (triiodothyronine) is an active functional hormone in the body and not T4 (thyroxine). It is necessary to convert T4 into T3 to act in the body. So try to modify the dose of Cytomel. There are many contributing factors that result in hair problems. Thyroid replacement, stress, and anxiety are some contributing factors. After taking the new dose for four weeks, you can repeat the thyroid profile and iron. You are taking a healthy diet and do not be worried.
