Hello doctor,

I recently had an ultrasound due to elevated alkaline phosphatase 231 IU/L, and C-reactive protein 85 mg/dL. Ultrasound found multiple lesions, with one lesion in the size of 3.94 x 2.05 inches with mild internal vascularity. Currently, I have fatigue and dull pain in my upper, middle, and right abdomen. I have been on birth control pills for 17 years and 40 mg of Atorvastatin for a year. Also, I was recently diagnosed with type 2 diabetes. I am waiting for a doctor's appointment but am wondering what the next steps would be regarding the investigation. I am concerned about the malignancy risk.

I am sorry to hear that you are experiencing these symptoms. Ultrasound suggests multiple lesions with vascularity. This could mean there might be HCC (hepatocellular carcinoma), hepatoma, or abscess but without CECT (contrast-enhanced CT scan) abdomen, it is difficult to tell. The next steps would be CECT abdomen, and if needed thorax plus some routine markers from blood as well as CBC (complete blood count) and renal function tests, serum calcium apart from alkaline phosphatase. Without knowing the specific nature of the lesions found on your ultrasound, it is difficult to determine the chances of malignancy. However, the fact that one of the lesions is quite large and has mild internal vascularity may raise concern for the possibility of cancer.
