Hello doctor,

I have symptoms like stricture urethra and difficulty to urinate, and I need to apply more pressure during urination and my penis is not circumcised. Nowadays, I crack my penis foreskin during contact, and sometimes, a smegma-type white substance is seen on the penis. I also have a burning sensation after semen loss.

I read your query and understand your concern. From your history, the condition is more likely a stricture urethra. For diagnosis, you need urethrogram antegrade and retrograde studies. After that only, we can decide on treatment options. As far as smegma is concerned, treatment depends on your ability to retract the foreskin. If you can completely retract the foreskin, you can clean the area and apply Canesten (Clotrimazole and Lidocaine) cream 1 % twice daily. If you cannot retract the skin, you will need circumcision to resolve the issue.
