Hi doctor,

I am a 23-year-old male. I do not have diabetes or hypertension, and I do not smoke. I had unprotected sex two weeks back. After three days, I had pain in my left testicle. Now, I do not have pain. I have mild pain after ejaculation. I also have redness and bumps on the foreskin of my penis. I had taken an STI test (PCR + HIV Combo) last week, and it was negative. Also, I have the urge to pee, and I frequently urinate. Kindly help.

I understand your concern. From the symptoms that you have described, you are likely to have urethritis with epi-didymo-orchitis, which may be a component of prostatitis. I suggest you answer the following question for assisting you further: Do you have any discharge from your urethra? Do you have trouble passing urine, like straining to pee, burning, or blood in your urine? Did the bumps and redness on your penis appear after having sex? Was the sex with your regular partner or someone new? Are you sexually active? Do you masturbate regularly? Do you have pain in the area between your testicles and anus? I also suggest you share the pictures of the affected area for further assessment.

Thank you for your reply.

I do not have any discharge besides pee and semen. I do not have any difficulties while passing urine. The bumps started with redness three to four days after sex, and it is still present. I masturbate four to five times a week. I have mild pain after ejaculation, and my precum is normal. However, there is some jelly-like ejaculation. I have pain in my pelvic region. I do not have pain in the anus. I have weakness in my upper body. Kindly help.

I understand your concern. From the images attached, you may have contracted genital herpes from your new partner (attachment removed to protect the patient's identity). However, not all images are very clear. I know you tested negative for the STIs, but sometimes, a test done at the wrong time may not pick up herpes. The red bumps should have been resolved by now if you did get herpes. The white bumps at the base of your penis head are normal variants. Herpes may or may not recur. I suggest you the following medication (consult with your doctor and discuss with him or her before taking these medications): Tablet Chymoral forte thrice a day for 14 days, capsule Doxycycline 100 mg twice a day for 14 days, and Surfaz SN (Beclometasone, Neomycin, and Clotrimazole) ointment. Apply this twice a day over your penis for seven days. I also suggest you avoid intercourse for at least one month and use a condom for preventing infections. I hope this helps you.
