Hello doctor,

I am a 19-year-old girl. I feel like urinating every minute and cannot sit calmly as my vagina has pain and itching deep inside with a strong urge to urinate. I have experienced this thrice in the last two to three months, lasting about three hours. Is it due to sexual intercourse, or can it occur if I am not involved in any sexual activity?

Pain and itching in the vagina are due to a vaginal yeast infection called candidiasis. It can cause urine infection, as it is very common in females. Therefore, I suggest you use the following medicines and start medicine after a urine test. If there is no improvement, visit the gynecologist for detailed evaluation, examination, and management.

Clotrimazole pessary (vaginal tablet) 100 mg, place one pessary daily inside the vagina at night for seven days.

Capsule. Fluconazole 150 mg, one capsule in the morning for one day.

Tablet. Ciprofloxacin 500 mg, one tablet in the morning and night for 14 days.

Cranberry sachet one sachet in the morning and night for 14 days.

Consult your specialist doctor, discuss with them and take medications with their consent.

Investigations to be done

Ultrasound KUB (kidney, ureter, and bladder), pelvis, and PMRV (post-micturition residual volume), urine test, urine culture sensitivity.

Treatment plan

Preventive measures

1. Drink eight to ten glasses of water daily. 2. Do not hold urine. 3. Maintain proper hygiene of genitals. 4. After urinating, clean your genital by washing it from front to back. 5. If married, then always urinate before and after sexual intercourse. 6. Do not wear tight pants or pajamas. 7. Do not wear synthetic underwear. 8. Use loose-fitting cotton clothes and underwear. 9. Do not use vaginal soaps, perfumes, and deodorants. 10. Avoid premarital and extramarital sex as it carries a high risk of STD (sexually transmitted disease) even with a condom or other protection.
