Hello doctor,

I am a 23-year-old female who is experiencing frequent urination with cloudy urine. I seem to have noticed that the walls of my vagina are very dry, and it is getting uncomfortably itchy that my sleep gets interrupted. What can I do to stop the itching and get my urination back to normal?

You are likely suffering from a urinary tract infection and possibly vulvovaginal candidiasis. Please send a picture of the buttocks region's lesions to confirm tinea or ringworm infection before medicines can be advised. Also, in the meantime, you can start using the alkaline vaginal wash, both internally and externally, to keep the vaginal area clean. Antibiotics and antifungals (vaginal pessaries and oral antifungals) would be required to be assessed after evaluating the lesions.

Please see the attached photos of the areas with concern. Pain is felt in between the butt cheeks and the skin in between the vagina and the anus.

I have seen the pictures (attachments removed to protect the patient's identity). This is a typical tinea corporis infection (fungal infection). It would require antifungal vaginal pessaries and oral antifungals with local antifungal cream application to tide over the infection.

1. Clingen Forte vaginal pessary per vaginum once at night for six days.

2. Tablet Itraconazole 200 mg once in the afternoon for five days.

3. Ointment Luliconazole 1% for local application over lesions thrice a day for 10 days.

4. Tablet Nitrofurantoin 100 mg twice daily for seven days (antibiotic for UTI)

5. Use alkaline vaginal wash twice a day for vaginal hygiene. Avoid intercourse for three weeks.
