My partner's UPT and beta hCG are negative. Does she need any other test to rule out pregnancy?

Patient's Query

Hello doctor,

My partner did a UPT after 24 days of last intercourse and a serum beta hCG test after 26 days. UPT negative and beta hCG 0.25 mIU/ml. Is it enough to rule out pregnancy? Or have we tested too early? If a further test is required, please guide us.

Answered by Dr. Reetika

Hi,

Welcome to icliniq.com.

I fully understand your concern and will surely help you. See, it is always better if the pregnancy test is done after any delay in periods. Nevertheless, 24 days is a good time for hCG to rise if the female has conceived, so it is fine. It will be helpful for me to advise you better if you can tell me the woman's last menstrual period date and cycle length. Also, whether the intercourse was unprotected or protected? Although based upon your explanation, yes, it is evident that the pregnancy is not there. However, to be sure, I request you to take a urine pregnancy test with the first urine of the day for an accurate result. Do not drink too much fluid before your test. Adhere to all guidelines written on the package of the pregnancy test kit. If the test outcome is positive, we may have to check again. Else for a negative result, we will rule out the possibility of pregnancy.

Patient's Query

Thank you doctor,

I had unprotected sex. I forgot to mention she did get her periods (bleeding more precisely), but it was five days before the expected date. The bleeding was as usual but suddenly increased heavily on the fourth day, and then it stopped after a couple more days. Also, I believe I had withdrawn before ejaculation. We did UPT early morning on the 25th day and serum hCG on the 26th-day post-sex. Is it conclusive? Or do you recommend any further tests?

Answered by Dr. Reetika

Hi,

Welcome back to icliniq.com.

As you have now told that she has experienced bleeding, however, it was a few days early. Did she take any emergency contraceptive pill or any other medications? It is evident she is not pregnant. If you wish, just do a UPT (urine pregnancy test) tomorrow morning as explained and let me know if it is positive. If negative then it is perfect.

Patient's Query

Thank you doctor for the reply,

We did the urine pregnancy test early this morning. It was negative. No, she did not take any emergency contraceptives. However, she has been taking oral contraceptives for menorrhagia for the past two months. Is this result conclusive? Can we rule out pregnancy completely, or is any further testing required?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Wonderful that the test is negative. Yes, with the early morning test coming back negative and having experienced a period after your intercourse, we can confidently rule out pregnancy. No further tests are required. When are you expecting your next period?

Thank you.

Patient's Query

Thank you doctor for the reply,

We are expecting her next period in around two weeks. I wanted to ask, does her taking contraceptives for menorrhagia in any way influence the pregnancy test results, or are they independent of each other? Can I let go of the stress completely?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Yes, do not worry. Please let go of the stress and get in touch after your next period.

Thank you.

Patient's Query

Thank you doctor for the reply,

My girlfriend and I got casually intimate a few days ago with no penetration due to concerns about precum. Afterward, she got her period. We decided to go for a beta hCG test, and the first result was 2.18 mIU/mL. We repeated the test, and the second serum beta hCG result was 2.28 mIU/mL. Additionally, she has taken three negative urine pregnancy tests in the meantime. Could she still be pregnant? Why is her hCG level increasing when it used to be under 1 mIU/mL (about 2 months ago when tested for another instance)? Please help.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Anything below 5 mIU/mL is indicative of non-pregnancy. As far as HCG levels are concerned, they start increasing or doubling, rising exponentially after the successful implantation of a fertilized egg. So, it is safe when it is below 5 mIU/mL. What is more important is whether there is a significant rise in it. A reading below 5 mIU/mL is fine as long as it is not showing an exponential increase every week or every few days, as it happens in early pregnancy. So, do not worry and stay happy.

Feel free to ask any questions or to share any additional information.

Thank you.

Patient's Query

Thank you doctor for the reply,

Yes, the second serum beta hCG test was conducted one week after the first one. The first result was 2.18 mIU/mL, and then, after one week, it was 2.28 mIU/mL. It has been over 5 weeks since we got intimate. Are these results conclusive for ruling out pregnancy, or do we need any further tests? We are seeking closure on this matter. Please suggest any additional tests that can completely rule out pregnancy in our situation. Also, why is the hCG level above 2 mIU/mL now when a few months ago, it was below 1 mIU/mL?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Do not worry. If the change in value from 2.18 mIU/mL to 2.28 mIU/mL within one week, it is conclusive evidence of non-pregnancy. Anything below 5 mIU/mL indicates non-pregnancy, and since she has already had her period, there is no need for concern. Furthermore, if conception had occurred a month ago, the hCG values would have been much higher.

Thank you. I hope your queries are resolved and any further queries are welcome.
