Hello doctor,

I see a distorted vision for the past one year. I see distorted shapes, especially when I tilt my head. Ten months ago, I visited my eye doctor. He did a dilated eye exam and said my eyes are normal. But I still see distortions. I had a lasik ten years ago. I can see clearly, but I also see distortions of the shape of things. Please help.

I have noted your history. You had lasik ten years ago. Are you sure you did not have any distortions in the image then or only for one year? What was your previous power before lasik? When lasik is done it flattens the cornea only for a certain diameter of the central cornea. The higher the power, the lesser the diameter. So if you look at an object at an angle, especially in the peripheral field, you might be observing through the ablated area's edges. This is more so if your pupil diameter is wide, which is common in myopes. Secondly, you have not mentioned if the distortion is in one eye or in both eyes. In the report you attached (attachment removed to protect patient identity), the left eye appears to have astigmatism. Astigmatism is when the cornea is not spherical but oval. This can also cause distortion. Thirdly, changes in the crystalline lens of the eye in advanced stages of cataract can cause distortions. Early cataract is common in people who have high myopia. Finally, a macular pathology can cause distortion of images. Your retinal examination a year ago was normal, according to that doctor. What I suggest is, if it is bothersome or increasing recently, get another check-up. Astigmatism, once corrected might resolve it. The doctor will also look for cataract or retinal pathology. They may ask for a scan called OCT (optical coherence tomography) of the macula to determine if it is a macular problem. I hope this helps.

It is most likely related to lasik and unstable tear film. Do not worry. You are not going blind. Dryness is very common and tends to increase after lasik, especially with the usage of computers these days. Continue using lubricating eye drops five to six times daily and add Omega-3 to your diet. It helps stabilize the tear film. You need to get your retina checked every year because of your history of myopia. The headache may be due to astigmatism that I mentioned. Use spectacles when working on a laptop or watching TV. Headache can also be due to stress. Take frequent breaks when working on the computer because you will get a headache when the eye muscles get fatigued.
