Patient's Query

Hello doctor,

I am suffering from vomiting for a long time. I had a consultation with an MD, and they gave me medicine for the same. They told me that I am affected by gastric problems and acidity. Still, I am suffering from the same problem with forceful vomiting. Sorry, I forgot their prescription, and I am regularly taking garlic capsules. Kindly give me a permanent solution.

Hello,

Welcome to icliniq.com.

I completely understand your concern and I know it can be very frustrating to suffer from repeated episodes of vomiting since long time. There are numerous causes of repeated vomiting since long like

1. Obstruction in esophagus, stomach, and intestines.

2. Chronic infection of intestinal tract.

3. Liver diseases.

4. Disease of gall bladder.

5. Disease of pancreas.

6. GERD i.e gastroesophageal reflux disease.

7. Diseases of heart.

8. Diseases of inner ear like motion sickness, etc.

9. Uremia- cancer of brain.

One thing is sure in your case that your vomiting is not due to acidity because if it has been due to acidity it would have been resolved when your doctor gave medicines for acidity and another thing is that you say vomit comes with full force but in acidity this type of forceful vomiting do not occur.

I need some more information about your clinical history so that I can make a precise diagnosis and find out the root cause of your problem so kindly provide me the following information.

Exactly when you first started getting vomiting episodes? How many times usually you get episodes of vomiting daily? Did you ever suffered from any major disease until now? Is you vomiting related to meals anyway i.e. vomiting after food? Does the vomit contains only fluid or other food particles? Did you ever noticed blood in vomit? Color of vomit? Any foul smell, burning sensation, or abdominal pain? Any weight loss in the last few months? Do you experience vertigo, dizziness, loss of consciousness, headache? Any vision disturbance? Any frequent loose motion or constipation?

Hello,

Welcome back to icliniq.com.

I have studied and evaluated your test reports and prescription slips in details very carefully (attachment removed to protect patient identity) and I have noticed some important things.

In your urine analysis report you have trace amount of proteins present in the urine which is not a good sign, actually in the normally functioning of kidneys all the protein are filtered out of blood and is reabsorbed back by the kidneys and thus normally there should be no presence of proteins in the urine. Let me explain, function of kidneys in humans is to excrete all the waste products out via urine and reabsorb the essential nutrients like proteins back from the urine so, when there is some problem with the normal functioning of kidneys then the reverse happens i.e what happens is that the waste products of body are not excreted and starts accumulating inside the body and essential nutrients like proteins starts getting excreted out via urine. So when there is some problem in the kidneys then the waste products slowly starts getting accumulated in the body and accumulation of waste products like urea, creatinine, etc., in the body can lead to episodes of vomiting. I am surprised that why your treating doctor did not notice this thing and did not get the tests to check the functions of your kidneys.

Another thing I have noticed is that your doctor has missed many other important tests which he should have advised you to find out the root cause of your problem. He just prescribed you medicines for acidity and thats it. This is not the right way to treat a patient.

You need to get some tests which are very important specially tests to check your kidney functions. So kindly get the following tests done from a good laboratory in your city.

1. RFT (renal function test). 2. LFT (liver function test). 3. Serum amylase.

4. Serum lipase.

5. Spot urine for microalbiminuria.

6. Ultrasound of abdomen and kidneys.

7. B.M.T i.e barium meal test.

8. CT scan of brain.

Once you get these tests done, kindly send me a scanned copy of your reports. Until you get your tests done, start the following treatment and make sure that you take your medicines on time and do not skip any dose.

1. Capsule Rabeprazole two times a day.

2. Tablet Ranitidine two times a day.

3. Tablet Domperidone 10 mg three times a day 15 minutes before meals.

4. Syrup Lupizyme two teaspoons three times a day just after meals.

5. Syrup Sucralfate two teaspoons three times a day after meals.

Start taking this treatment from tomorrow morning and send me the reports of tests I mentioned above.

Hello,

Welcome back to icliniq.com.

Well, I have seen all your reports (attachment removed to protect patient identity), and I am happy that all your reports are normal. I was worried for your kidneys but your kidney reports have come normal and so do not worry at all. Both of your kidneys are fine.

Uremia is not cancer. Uremia is a disease of kidneys but as I said your kidney reports are normal. So you do not have uremia. Start taking the medicines which I told you in my last reply for one month and if you feel any problem you can contact me any time you want.

Same symptoms don't mean you have the same problem. Consult a doctor now!
