Hello doctor,

The patient has diabetes for the past eight years, and a year back, she had mild chest pain. However, ECG and other tests were normal. Rarely, she gets mild chest pain and subsides by itself. She also notices heavy breathing while climbing. Her random sugar level is 206 mg/dL, and her BP is 143/83 mm of Hg. Do you recommend any tablet for chest pain? We are planning for the COVID vaccine. Do you recommend the same for her? What are the clinical tests (like antibodies) that you recommend before having the COVID vaccine? Which vaccine do you recommend (live or non-live)?

Thank you for the query. She is diabetic and hypertensive and has chest pain, so if resting ECG (electrocardiogram) is normal, then echocardiography should be done at rest and with stress. Another option is myocardial perfusion scanning (MPS) for inducible ischemia to confirm the presence or absence of ischemic heart disease. She can get the COVID-19 vaccine, and no test is needed before having the COVID vaccine. I suggest a non-live vaccine. Regards.

Hello doctor,

I had attached the report of the echocardiogram taken last year when she developed mild chest pain. We are scared to go to the hospital to echo due to increased corona cases across the city. Do you suggest any medicine for this? Or can she take tablet Nitrocontin and tablet Ecosprin till I get the following report? Also, she feels exhausted and weak after cooking and daily chores. She is taking tablet Shelcal and tablet Renerve plus in the morning. Do you suggest any other medicine to improve her health?

According to your report (attachment removed to protect the patient's identity), echocardiography and blood tests are normal. Please send her a recent ECG. If ECG shows any abnormality, then echocardiography should be done. Also, if breathlessness on climbing stairs is of recent onset, then echocardiography should be repeated along with ECG.

She do not have any recent ECG. She has to go to the hospital, but we are scared to step out due to lockdown and increased corona cases across the city. Can she continue tablet Ecosprin and tablet Nitrocontin? Any tablet do you suggest for weakness and tiredness after daily chores?
