Hello doctor,

I surgically removed my lower left wisdom teeth five days ago. I still have a yellow liquid mixed with saliva that tastes a bit sour keep coming out from the surgical site. I do not have any fever, numbness, or pain. Do I have an infection?

Since it is already five days, you had the extraction. By now, you should be entirely relieved from all symptoms. I would like to see the surgical site. Kindly upload an image of the area, and I will let you know for sure whether any infection or any other reason.

The image was very clear (attachment removed to protect patient identity). Since the extraction was surgical, it takes time to heal. Inflammation is common, secondary bleeding (minute amount) is noted sometimes. That makes the saliva yellow.

Some other causes are:

- Irritation of the blood clot.

- Consumption of hot food or beverages.

- Difficult or prolong extraction leading to severe inflammation (swelling).

So I advise capsule Sporidex 500 mg (Cephalexin) one capsule BD for five days. Take tablet Ibuprofen one tablet BD for five days, capsule Omeprazole BD before meals for five days, tablet Ethamsylate one tablet sos (blood clotting agent). It will pause if there is any minute bleeding.

Start the medications. If any secondary infection persists, I have already prescribed antibiotics. So continue these medications for the next five days.

Use Betadine (Povidone Iodine 2%) mouthwash, gargle in lukewarm water two to three times a day.

Vitamins in high doses can result in altered taste sensation. Your clinician has prescribed the vitamins to reduce the inflammation post-surgery. But such a high dose is not needed. Try placing an ice pack outside the mouth if swelling is noticed. I suspect the fluid is blood in very minute amount, try placing ice cream over the area. Ice helps in clotting.

Do not apply tongue over the area, or the clot may dislodge and initiate bleeding again. Brush twice gently, but avoid the extracted site. Avoid Sticky or sweet food. Gargle with mouthwash will clean the area, but do it gently and do not rinse forcefully.

If you follow these instructions, I am confident you will see the difference within two to three days.
