Patient's Query
Hello doctor,
I am four weeks pregnant. For the last four days, I have been on bed rest and was given Misoprostol 15 hours after taking the abortion pills. I only noticed a small amount of blood twice after that, and since then, there has been no bleeding.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I can understand your concern.
At four weeks of pregnancy, if you took Misoprostol orally and only had a little bleeding twice, then nothing after that, it suggests the abortion has not worked fully. Normally, with a proper regimen, you would expect cramping, moderate bleeding, and passage of tissue.
What you describe is too little. That means the pregnancy could still be continuing, or retained inside. It is not safe to just wait blindly, because an incomplete abortion or an ongoing pregnancy is possible.
The probable causes: Incomplete abortion or failed medical abortion due to incorrect regimen or insufficient effect of the tablets.
Investigations to be done:
Urine pregnancy test after one week.
Blood beta hCG (human chorionic gonadotropin) to see if levels are falling.
Ultrasound of the pelvis to confirm if the sac is still present or cleared.
Differential diagnosis:
Incomplete abortion.
Ongoing pregnancy.
Delayed response to Misoprostol.
Probable diagnosis: Failed medical abortion.
Treatment plan:
You must not repeat tablets on your own.
Visit a gynecologist urgently, they will check with a scan.
If products remain, they may advise repeat medication under supervision or a minor procedure (manual vacuum aspiration).
Do not ignore, because retained tissue can cause infection and heavy bleeding later.
Regarding follow-up:
You need to get an ultrasound and beta hCG soon, then update me with the report.
Please follow up with your gynecologist directly for complete care.
Preventive measures:
Always take abortion medicines only under direct medical guidance with the correct combination (Mifepristone plus Misoprostol).
Never self-administer without a prescription, as it often fails and can be unsafe.
I hope this information will help you.
Kindly follow up if you have more concerns.
Thank you.
