Is prolonged bleeding after a Misoprostol abortion normal?

Hello doctor,

I have a medicated abortion in the 2nd week using Misoprostol. Today is the 15th day of taking medicine, and bleeding has slowed down but has not yet completely stopped. What should I do to completely stop the bleeding? Is there any medicine for this?

It is normal to experience light bleeding or spotting for up to two to three weeks after a medication abortion with Misoprostol. Since it has been 15 days and you are still bleeding, here is what you should consider:

1. Is the bleeding normal or a concern?

Normal if:

a. Bleeding has gradually decreased (from heavy to light spotting).

b. No severe pain, fever, foul-smelling discharge, or heavy clotting.

Seek medical help if:

a. Bleeding remains heavy (soaking more than two pads per hour for several hours).

b. You have severe abdominal cramps, fever, chills, or nausea (possible infection).

c. Large clots or tissue continue to pass.

d. Symptoms of incomplete abortion (persistent bleeding, pain, ongoing pregnancy symptoms).

2. Methods to stop the bleeding faster

a. Rest and hydration - Avoid strenuous activities and drink plenty of water to help your body recover.

b. Iron supplements - If bleeding is prolonged, consider taking Ferrous sulfate (iron tablets) to prevent anemia (low red blood cell count).

c. Tranexamic acid (TXA) - Under doctor’s advice. Dosage: Typically 500 mg–1,000 mg, two to three times per day for up to five days (only taken under medical supervision).

d. Check for incomplete abortion - If bleeding does not stop soon, a doctor may suggest an ultrasound to check if any tissue remains. If retained tissue is present, a dilation and curettage (D&C) procedure or additional Misoprostol may be needed.

3. What should you do now?

a. If bleeding is light and slowing down, monitor it for a few more days.

b. If bleeding remains persistent or heavy, visit a gynecologist (OB-GYN) for an ultrasound to check for retained tissue.
