When I was pregnant, I had a surgical abortion eight years back. My parents have chosen a guy for me to get married. I do not want to see a doctor, as my fiance will get to know about my past pregnancy and abortion because the doctor is well known to my fiance.

Well, I am deeply concerned about your worries. Let me explain to you that once you have a pregnancy or terminate the pregnancy, whether surgical or medical termination, positive results are shown for a maximum of three weeks; after that, the result becomes negative.

There is no beta hCG (human chorionic gonadotropin) or ultrasound that can detect the previous pregnancy. Beta HCG remains elevated till the pregnancy is positive after the level becomes less than 5 IU (international unit). I assure you that no medically proven investigation can detect that you had a surgically aborted pregnancy. But your fiance can identify that you had sexual intercourse before him because of losing your virginity, and that one you can make him understand.

Once again, I want to make sure there is no medical way to prove whether you had a pregnancy before or not; only we can confirm by asking about the patient's history. No, all these tests will not detect the previous pregnancy. No test tells us this patient had a history of previous miscarriages that may have been surgically or medically aborted. We can confirm that only by taking a history from the patient’s past obstetric history. These investigations that I wrote for you are routinely done in all women presented with pregnancy. I just said once you get pregnant in the future, you have to do an antenatal checkup and carry out all these tests. High-risk pregnancy is pregnancy associated with complications like anemia in pregnancy, hypertension in pregnancy, diabetes in pregnancy, gestational diabetes, SLE (systemic lupus erythematosus) in pregnancy, thyroid in pregnancy, and epilepsy in pregnancy.

Nobody will know your previous history of surgical abortion; maybe only you and your previous fiance will know. It simply depends on the answer you give, and he or she will move to the next question. Hide that, which is good for you, and it should not affect your current family. Just say no; this will not affect or change the management he is going to do. So do not worry. I assure you, until you share your past history with anyone, nobody will come to know your past record.   1. 7 to 9 weeks: Dating scan.

None of these tests are intended to detect a past surgical abortion, so do not worry about it. No one will know unless you share your history. Do not get overly stressed. Well, yes, a pelvic ultrasound is a definitive test to assess the uterus and ovaries.

1. Uterus: We check the endometrial thickness, regularity, and any endometrial polyp or fibroid first. Myometrium: We check whether the fibroid is there. Serosal layer: We assess for any subserosal fibroids.

2. Ovaries: The size of ovaries and cysts, any free fluid in the pouch of douglas.According to your ultrasound report, everything was perfect; only there were polycystic ovaries, and that Is not a big problem.

Polycystic ovaries: It is a common problem for young girls.

You have to do lifestyle modification.

  1. Stop eating junk foods like burgers, pizza, and cold drinks.
  2. Start doing exercise 3 days a week for a 30-minute walk.
  3. Try to reduce weight.

Along with that, I will advise you on these medications.

  1. Tablet Metformin 500 mg 2 times a day for 4 months. This will help you to reduce the weight and glucose absorption. You may have nausea, vomiting, and diarrhea; it is the side effect of the drug. Do not worry about it
  2. Tablet Folic acid 5 mg is good for preconception. One tablet at night for 3 months. It will help you to get pregnant once you get married.

Once the patient had a cesarean section delivery, then we called scar uterus for abortion; there will be no uterine scar at all. Now you do not have any problem that can tell your previous pregnancy. I assure you there is no need to worry at all; there is no way they can figure out you had a pregnancy before. However, you should make sure that you do not expose your past history to anybody. I guarantee nobody will come to know.
