Hello doctor,

My 3-year-old daughter was diagnosed with accommodative esotropia. At the appointment, she cried intensively and constantly moved her head, making the doctor's job hard to see. One of her eyes does cross a little, but for the most part of the day, you will not see it happen. She was prescribed a +4 on each eye. She is using the glasses now and doing fine, but my instinct tells me something on that appointment could have been misdiagnosed because of her temperament. Is that possible? I do have a second appointment for a different doctor scheduled one week from now. I am concerned that the glasses will make her eyes worse(they do not cross much you will see it happen more once when she is tired for a nap). Vision therapy was not a possibility, in his opinion. If it is a developing issue, my question is, why not? I am scared that we got her on glasses too fast. Have there any cases where the children have very, very mild accommodative esotropia and will outgrow without glasses?

I totally understand your concern. I saw the pictures (attachments removed to protect the patients' identity) you uploaded. In the pictures, the left eye is esotropic. See what happens in accommodative esotropia: when the person sees a near object for too long or if the muscles have fatigued like by evening, the eye will cross. It might not be visible all the time, but if it is happening even once it is a condition, then. A number of plus 4 is fine for her age. The power will decrease as her eyes grow till the age of 8 years. Also, a total no to mobiles, I-pads laptops anything which requires a digital screen will only aggravate her condition. You got her diagnosed at the right time. If you had delayed, her eyes would be permanently squinted and vision permanently weak. So please make her wear the glass at all times so that her vision develops to 100 percent. It is very important. Wait till she is six years of age, and then we will see how much the squint is. If it is less than 20 degrees, then only glasses will be required. If it is more, she might require cosmetic surgery. But that is in the future. For now, glasses are the only option you have. And no gadgets. Any further query I will be happy to help.

Hello doctor,

How about close activities like coloring or writing? Will that make it worse since she is looking closer? She loves TV, and we cut down the time she is on it, but when she is watching TV, at what distance should she be? Should the TV be up higher or level with her eyes? She complains about not seeing anything far away. She has been wearing glasses all day for three days only. Will she eventually be able to see clearly far away in 2 to 3 weeks?

Writing and coloring will not have any harmful effects. She can indulge in them as much as she likes. The lighting of the room should be proper, and the distance between her and the table should be 30 cm. For TV, the distance should be more than 3 feet. She is not able to see far away now because of her high power. I will explain it in a simple way. Light has not been focused on her retina because of the plus power, so everything she sees is blurred. Till now, her brain does not know it can see clearly. Now once she starts wearing glasses, a clear image will form on the retina, which her brain will perceive. Then her binocular vision means 3D vision will develop, which has not formed till now. So you will have to be patient. The good thing is it has been timely diagnosed, so treatment is very much possible.
