Hello doctor,

I just need help diagnosing my acne. It is like a white pus, but it is deep under a layer of skin on my left cheek. It can only be seen when I purposely stretch out that skin. Anyway, I picked at it a lot to try to get the pus out but the pus always comes back even when I clean my face all the time. And I have created these not very deep yet ice-pick scars. I need help trying to get rid of them. Will retinol and a dermatologist roller help get rid of it?

Post acne scars are three types: box scar, icepick (pin point scars), and rolling scars. Pinpoint scars are difficult to treat while box scars are easy. No creams or tablets can help you out with scars. The best technique is CO2 fractional laser and subcision. It takes five to six sittings but results are better than any other procedure. Dermaroller and subcision are also other methods but less effective than above. Filler has a hiding effect, and it fills the pits and gives an impression of fullness. It is not permanent. Chemical peels if scars are not much prominent then peels also could be effective. These are techniques you can go through. All procedures take four to six sitting for five to six months for better results. Pimples are somewhat hormonal and due to oily skin. I suggest the following measures:

1. Do not use hair oil and any sticky cream on the face.

2. Avoid oily food, chocolate, and milk products.

3. Avoid dust and use sunscreen preferably above 30 SPF (sun-protection factor).

4. Do not rub any cream on your face.

5. Do not use any steroid creams.

6. Continue treatment for at least a month.

7. Face wash or soap cannot provide treatment and is not very useful.

8. Do not touch acne and avoid pressing it.

9. For treatment, use Clindamycin gel. This is more preventive and curative so I tell patients to use it all over their face in the morning. Adapalene gel can be used over acne at night, and a tablet of Doxycycline 100 mg once a day.
