Does Neurofen makes Amoxicillin less effective?

Hello doctor,

I have a sinus infection and I have been taking Amoxicillin for five days so far. However, the pain has not gone away. I have been taking Neurofen sometimes when it is too painful. Is Neurofen the reason the Amoxicillin is not working effectively?

I understand your concern.

Sinus infections are notorious for harboring resistant bacteria and viruses. Acute sinusitis and acute or chronic sinusitis require potent antibiotics and also, medications for enhancing sinus drainage. Ibuprofen present in Neurofen is a nonsteroidal anti-inflammatory drug (NSAID) for general pain relief and not specific for upper respiratory and sinus-related pain. This drug does not cause any interference in the Amoxicillin (antibiotic) action as well. Do you also have a fever, runny nose, and throat pain?

For affirming sinus infection, you need a visit to an ENT specialist (otolaryngologist), and if possible undertake bacterial culture and sensitivity tests and viral detection tests of swabs from the throat and nose. Additionally, an X-ray of the paranasal sinuses would give the doctor a better picture of the possible sinusitis. Till then, you could take a

1) Combination of Paracetamol, an antihistaminic and a decongestant.

2) Steam inhalation twice daily.

3) Warm salted water gargles twice daily.

The Probable Causes: Viral or bacterial.

Investigations to be done:

  1. Swabs- throat.

  2. Nose- Bacterial Culture and sensitivity.

  3. For influenza A/B or RSV (Respiratory Syncytial Virus) viruses.

  4. X-ray paranasal sinuses.

Differential diagnosis: Influenza.

Probable diagnosis: Acute infective sinusitis.

Treatment plan:

1. Tab Sudafed/ Sainsbury's Max strength cold and flu (Paracetamol 500mg, Phenylephrine 1 mg, Caffeine 25 mg) one tablet twice daily for five days.

2. Tablet Allevia/ Allegra / any brand (Fexofenadine 120 mg) one tablet once daily for seven days. 3. Antibiotics (Amoxycillin or other antibiotics) to be started after investigation results.
