It appears to be atopic eczema with an itch mite hypersensitivity reaction. Here are some of the instructions for you to follow:
Apply Mometasone furoate (anti-inflammatory) cream or FM derma ointment (Fluticasone and Mupirocin) at night for 10 days.
Use Cetrimide soap (antibacterial) or Permite BB (Permethrin) bar soap for baths for two months.
Take Tablet Teczine-M (Levocetirizine ) once at night after food for seven days.
Avoid scratching and rubbing of lesions.
Keep your nails short.
Avoid known triggers.
Avoid extreme weather conditions.
Avoid synthetic wear.
Wear loose, comfortable, and thin cotton clothes.
Maintain proper hygiene, as hygiene plays a very important role in avoiding recurrence.
Wash clothes separately in hot water.
Tumble dry it.
Avoid sharing personal belongings.
Apply 5 percent Permethrin cream at night on day one and day seven (below the neck and all over the body). The contact period is eight to 12 hours, and wash it off the next morning.
Also, I want to learn more about your medical history, such as whether you have ever experienced abdomen or joint problems. Is there any history of travel, toothache, fever, exposure to dust, pollution, animal dander, food allergies, or any drug allergies?

There has been some improvement; I have attached some pictures so you can see, and she wants to start applying Nivea cream to evenly blend the complexion.

I suggest you follow the instructions mentioned below (attachments removed to protect the patient's identity).
1. Avoid exposure to dust, pollutants, animal dander, allergens, and pollens.
2. Apply Mupirocin (antibiotic) cream twice daily for two weeks (on lesions).
3. Stop steroid use, as prolonged use of steroids causes depigmentation or atrophy.
4. Apply Lacto calamine (antipruritic) lotion three to four times daily for 10 days.
5. Avoid scratching and rubbing lesions, as it may cause deep scars.
6. Take a tablet of Loratidine (antihistamine) 10 mg once daily after breakfast for 10 days (if itching).
Kindly consult a specialist, talk with them, and take medications with their consent.
