Hi doctor,
I was taking a combined oral contraceptive, specifically Trust pills. Now, I switched to Charlize, and I already took my first dose, which has a higher dose of Levonorgestrel. Do I need to have extra protection?

Since you have switched from Trust pills to Charlize, which both contain Levonorgestrel, Ethinyl estradiol, and Ferrous fumarate, both types of pills are combined oral contraceptives (COCs) with similar hormones but slightly different doses; the protection remains intact if you started Charlize right after finishing the Trust pack without missing a day. No extra protection is needed in that case. You are currently having a mild hormonal adjustment period due to switching between different COC brands.
But if there was even a single day gap between stopping Trust and starting Charlize, you should use condoms or avoid sex for the next seven days just to be safe.
You do not require any investigations unless you notice breakthrough bleeding or irregular spotting.
In your case, the differential diagnosis will be:
Hormonal fluctuation due to a change in formulation.
Missed pill–related ovulation risk.
Gastrointestinal upset affects absorption.
I would suggest the following treatment plan:
Continue to use Charlize daily at the same hour.
If no pills were missed and you started right away, you are covered.
If delayed by more than 24 hours after the last Trust pill, then use a backup (condom) for seven days.
Watch for mild spotting or nausea during the first month; this is normal with a brand change.
As a preventive measure, always start the new pack immediately after the last active pill of the previous one.
Avoid taking hormone-free breaks longer than seven days.
Try to maintain the same pill-taking time daily to stabilize hormone levels.
If you get any unexpected spotting, delayed withdrawal bleed, or pregnancy symptoms, let me know or repeat a home pregnancy test. Otherwise, continue the new pill normally.
