How to treat black stools in toddlers?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My toddler is three years old. He has been experiencing black stools and a bloated stomach for about four weeks now. I am worried and do not know what to do.

Please guide me.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

Your 3-year-old child has been experiencing black stools and a persistently bloated stomach for the past four weeks. This is not normal in toddlers and requires urgent medical evaluation. Here are some possible causes:

  1. Bleeding in the upper digestive tract (black, tarry stool called melena, which can arise from the stomach, esophagus, or small intestine).
  2. Iron, Bismuth medications, or foods like blueberries can darken stools, though these usually do not explain persistent bloating.
  3. Parasitic or intestinal infections can cause bloating and stool changes.
  4. Liver disease, malabsorption, or other gastrointestinal problems might cause bloating with stool changes.
  5. Serious conditions like intestinal obstruction or ulcers need to be ruled out.

I would suggest some investigations:

  1. Abdominal ultrasound.
  2. Complete blood count (CBC).
  3. Erythrocyte sedimentation rate test (ESR) to check for inflammation.
  4. Stool analysis and stool culture for ova and parasites.

I would like to review the investigation reports before advising specific treatment. Please follow up with me once you have the results, and also consult a pediatrician in person for a physical examination.

I hope this helps you.

Thank you.
