How to manage treatment-induced menopause in breast cancer?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am 51 years old and just finished chemotherapy for breast cancer three months ago, but now I am dealing with severe menopause symptoms from the treatment. The hot flashes are so intense that they happen 20 to 30 times per day, and I am soaked in sweat constantly. My oncologist says I cannot take hormone replacement because of my estrogen-positive tumor, but the symptoms are ruining my quality of life. Developed severe vaginal dryness and pain during intimacy that is affecting my marriage.

Also, I'm having terrible mood swings and depression that might be from the tamoxifen I started. My bone density scan showed osteopenia, which is concerning for fractures. Sleep is impossible because of night sweats and anxiety about cancer recurrence. The chemotherapy also caused neuropathy in the hands and feet, which makes daily tasks difficult.

What breast cancer treatments are available for managing treatment-induced menopause? Are there safe alternatives to hormone therapy?

Kindly help.

Hi,

Welcome to icliniq.com.

I can understand your concern.

I am so sorry you are facing this storm after everything you have been through. You have shown incredible strength, and I promise we have good options that keep you safe while easing these symptoms. For hot flashes and sweating, low-dose Gabapentin (also helps neuropathy) or Venlafaxine (an antidepressant) can cut flashes by half or more. Simple tricks like a bedside fan, cotton layers, and sipping cold water at night help too.

  1. For vaginal dryness or pain, non-hormonal hyaluronic acid gels are used two to three times weekly to rebuild moisture safely.

  2. Vaginal laser therapy can also heal delicate tissues; many patients find huge relief.

  3. For intimacy, silicone-based lubes reduce friction pain instantly.

  4. For bone health and mood, daily weight-bearing walks (even 10 minutes) protect your bones.

Let's check your vitamin D supplements, as calcium is key. If Tamoxifen worsens mood swings, ask your oncologist about switching to Exemestane if you are now postmenopausal. Short-term CBT (cognitive behavioral therapy) helps both mood and the fear of recurrence.

For neuropathy and sleep, acupuncture may ease nerve pain and flashes. For sleep, try cooling mattress pads and mindfulness apps before bed. You can feel like yourself again. Start with one step, maybe the vaginal gel or Gabapentin, and we will adjust together.

I hope this information will help you.
