Hello doctor,

My father has been dealing with chronic sinusitis and underwent functional endoscopic sinus Surgery (FESS) twice once in 2017 and again in October 2024. His doctor recommended daily nasal washes, which he has been following. However, he feels the current treatment is not as effective as expected. Recently, based on someone’s suggestion, he is considering trying homeopathy.

Given that this is not the initial stage of his condition and he has already undergone two surgeries, would homeopathic treatment be effective? Or should we continue with the current treatment plan?

Chronic sinusitis is an immune-mediated condition in which the body's immune system produces inflammatory markers in the mucosa of the paranasal sinuses in response to normal environmental antigens.

The most effective treatment for this condition is steroids. However, long-term use of oral steroids is not feasible due to their potential side effects. Instead, nasal steroids, delivered through nasal washes or sprays, are commonly used.

In cases of severe mucosal inflammation, the mucosal thickening can block the sinus openings, preventing nasal steroids from penetrating deeply into the sinuses. A surgical procedure called functional endoscopic sinus surgery (FESS) is performed to address this. This procedure removes thickened mucosa and polyps, widening the sinus openings and allowing the nasal steroids to reach deeper into the sinuses.

Even after FESS, patients must continue using steroid nasal washes (which have minimal side effects compared to oral steroids) for an extended period, often up to three to four years. This helps control inflammation and prevents the recurrence of sinusitis.
