Hello doctor,

I have noticed wearing off on the soles of my feet as I walk around a lot (hiking). I am concerned if it is severe and if will it worsen or heal over time. I have also attached the images. Kindly help.

I can understand your concern.

From the images sent by you (attachments have been removed to protect the patient's identity), It appears to be corn due to chronic friction. Localized hyperkeratosis with scaling is noted on the foot. It can be caused due to repeated mechanical trauma to the feet from poorly fitting footwear or anatomic defects. To be more precise, it is soft corn. Differential diagnosis is keratosis punctate.

I advise you to follow the below-mentioned instructions:

  1. Avoid mechanical stress or chronic friction.
  2. Wear comfortable footwear or sandals.
  3. Soak the leg in lukewarm water and then apply topical 6 percent(Clobetasol) ointment or lotion on the affected area at night for three to seven days.
  4. Moisturize the skin of the foot with foot care cream like U-Lac cream (Urea, Lactic acid) or liquid paraffin lotion or emollient, two to three times daily for 10 days.
  5. Wear ortho-wear or soft footwear.

I would like to know the answers to the following questions.

  1. Have you been using tight footwear?
  2. Can you feel it painful on vertical pressure?
  3. Do you have a habit of sitting with the foot tucked under the body with or without a prominent underlying talus?
  4. Is hiking your occupation?
  5. Is there any joint pain?

The well-defined, round, and rough surface is observed which confirms it to be soft corn. I advise you to follow the below-mentioned instructions.

  1. Maintain proper foot hygiene.
  2. Avoid injury, trauma, or friction to the forefoot.
  3. Salactin paint (Salicylic acid and Lactic acid) with petroleum jelly can be applied for fast results.
  4. Salicylic acid of 40 percent can be also used with a cotton tip applicator after cleaning or soaking the foot in lukewarm water.
  5. If in case of severe pain, a small amount of intralesional steroid can relieve pain.
  6. Avoid ill-fitting footwear.
