Hello doctor,

I have had this lesion on my face for over 3 months, it has not gone away. My doctor injected it with Cortisone and sprayed nitrogen oxide on it, and it has not helped. He is not sure what it is but thought it could be lichen planus or lupus. However, those treatments have not been effective.

If a biopsy was done, then it should give you the exact diagnosis. At present, since you have undergone the procedure, the image of the lesion must have been altered. In lichen planus histopathology, basal cell degeneration and interface dermatitis with colloid bodies are the main findings. Whereas in lupus vulgarism, epidermis hyperplasia, tuberculoid granuloma with lymphocytes, plasma cells, epitheloid cells, giant cells, and fibrosis in the dermis would be classical findings. Lupus commonly sits in the face. At first, it appears as a small nodule joined to form plaque and gives an apple jelly appearance on the diascopy.
