How to check for abnormal eye movements?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I want to talk about my son, and I am sharing a video with you.

Please tell me whether you see any abnormal eye movements in this video of my son following my finger with his eyes.

I observed some unusual eye movements; he complained that he could not see well out of his right eye this morning. He could only see how many fingers I held up when his other eye was covered.

Hello,

Welcome to icliniq.com.

I understand your concern.

This is not how we can check eye movements.

The child should be in a sitting position with a fixation point. He should not move his neck or head while seeing; only his eyes should move.

Other than that, counting fingers is a vague examination. What you can do at home is ask your child 6 feet away from the tv/iPad. You can download a digital Snellens chart and ask him to read with either eye, keeping other eyes closed.

I hope this helps.

Thank you.

Patient's Query

Hello doctor,

Did you watch the video? Are you able to determine anything from looking at it?

Please let me know.

Hello,

Welcome to icliniq.com again.

Yes, I did watch the video (attachment removed to protect the patient’s identity)—twice. His right eyes seem to lose focus momentarily and look esotropic. But seriously, I cannot comment on his position—lying down, half-lids covering his eyes—and I cannot determine all his muscle actions or movements.

You can take the same video with him sitting on a chair. that would be of proper help. and check the vision, as I advised earlier.

I hope this helps.

Please reach out to me again if you need a follow-up.

Thank you.
