Will immunotherapy work in my mom’s PD-L1-positive cancer?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am asking abt my mother, she is 65 and diagnosed with stage 4B endometrial cancer with spread to the lungs. Biopsy showed endometrioid type, PD-L1 came positive, and MSI-high on report.

The oncologist suggested immunotherapy. We are trying to understand expectations. Her CA-125 is 210, and she feels weak but stable. We are worried abt side effects, as well as the benefits. Kindly let me know the following:

  1. Can stage 4B endometrial cancer respond to immunotherapy at 65?

  2. Does MSI-high mean better response chances?

  3. Is this treatment mainly for disease control, or can it shrink tumors significantly in some cases?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query.

I understand how overwhelming and frightening this must feel for you. Hearing that your mother has stage IV endometrial cancer can bring a lot of fear and uncertainty, and your concern for her is completely valid. I want to gently reassure you that even at this stage, there are still treatment options that can help control the disease and support her quality of life.

Since she is PD-L1 (programmed death-ligand 1) positive, this gives an important opportunity to use immunotherapy. These treatments work by helping the body’s own immune system recognize and fight the cancer. In many patients, this can slow the disease, prevent further spread, and, in some cases, even shrink tumors. Targeted therapy may also be considered alongside this approach, depending on her overall condition.

You mentioned the spread to the lungs, and it is natural to worry about that. Immunotherapy can still make a meaningful difference here by improving how the body responds to cancer. While every patient responds differently, there is definitely a reasonable chance of disease control and even reduction in tumor size.

Her CA-125 (cancer antigen 125) level is not very high, which can sometimes be a relatively reassuring factor, although we always look at the full picture rather than a single test value.

At the same time, focusing on her comfort and strength is just as important as treating the cancer. Making sure she is eating well, staying hydrated, and feeling supported emotionally can have a big impact on how she copes with treatment. Pain control should be taken seriously, and her medications should be given regularly so she remains comfortable. If she needs any additional supportive care, it should be provided without delay.

The plan going forward would be to continue with immunotherapy (a cancer treatment that boosts the body's natural defenses to fight cancer) along with best supportive care, and to review her progress after about a week to see how she is responding and adjust the plan if needed.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
