Hi doctor,

I suffer from severe and chronic eczema, starting from my neck and going down to my ankles. My skin scales every day and itches every minute. I am suffering because no pills, steroids, or prescription has helped me in the last 10 years. Please help me.

I understand your concern.

It is called erythroderma, when the eczematous patches involve more than 80 % of the body's surface area. In these cases, a first biopsy has to be done to confirm the diagnosis. It would help if you had proper investigations and were put on immunomodulatory therapy via Methotrexate or Cyclosporine. You cannot keep on taking steroid medication for the long term as it will have side effects. Steroid-sparing agents can help you in your case as they are of higher medication that can be prescribed only via direct physical consultation.

So what do immunomodulatory therapy, Methotrexate, and Cyclosporine entail? Are they medication or creams? How long will I have to be on that treatment, and will it help with my itching and pigmentation loss?

They are oral medications and you need a course of atleast three to four months course you need to do and it can help you with itching and pigmentation too and any how you need to continue with moisturizer creams even after resolution to prevent recurrence.
