My baby is seven weeks old. She is refusing the nipple and is feeding only for six to 10 minutes.

In a seven-week-old infant, feeding only for six to 10 minutes with nipple refusal can sometimes be benign (for example, the infant becoming more efficient or developing a mild feeding preference). However, in your baby’s case, it is concerning because the reported weight (4 pounds at seven weeks) is significantly below the expected weight for age, suggesting poor weight gain or failure to thrive.
Possible causes include inadequate milk transfer, latch or nipple issues, oral conditions (such as oral thrush or tongue-tie), gastro-oesophageal reflux, cow’s milk protein intolerance (if formula-fed or mixed feeding), intercurrent illness, or underlying systemic problems, especially if the baby was preterm or had a low birth weight at birth.
A baby who consistently refuses feeds or feeds for a very short duration may not be meeting caloric needs.
I strongly advise an urgent in-person evaluation to assess hydration status, weight trends, feeding technique, and the oral cavity, and to review the birth history and gestational age. If the baby has reduced urine output (less than six wet diapers per day), lethargy, persistent vomiting, fever, breathing difficulty, or worsening feeding, this should be treated as an emergency.
Until reviewed, continue offering feeds frequently (every two to three hours), ensure proper positioning and latch, and do not delay medical assessment given the low weight.
