Hi doctor,

I am a 30-year-old male, and for the past year, I have been experiencing involuntary muscle contractions in my neck and right hand. My neck sometimes twists to one side uncontrollably, causing pain and stiffness, and my hand occasionally clenches or spasms during writing or typing. These movements worsen with stress and fatigue and improve slightly with rest. I also notice mild tremors in my fingers and occasional cramping in my jaw while speaking.

My neurological exam showed abnormal posturing and increased muscle tone in affected areas. MRI of the brain was normal, and routine blood tests, including electrolytes, liver, and kidney function, were within normal limits.

My family history is unremarkable for movement disorders. I have tried physical therapy and oral medications like anticholinergics, but the improvement is minimal.
