How to manage severe GERD with grade C esophagitis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 42 years old and have been suffering from severe GERD for two years. It is significantly impacting my quality of life. I experience heartburn and regurgitation daily, especially at night, when acid rises into my throat, causing choking and coughing fits. My recent upper endoscopy showed grade C esophagitis with multiple erosions. My pH study confirmed severe acid reflux with a DeMeester score of 68.

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Based on your description, you are experiencing severe acid reflux, which is also supported by the endoscopic findings. The majority of acid reflux cases are due to a sedentary lifestyle and high body weight. If you are experiencing the same, then you should make some lifestyle changes.

For your current complaints, follow the instructions below:

  1. Take one tablet of Rabeprazole 20 mg by mouth once daily for 14 days.

  2. 14 days and a follow-up afterward to assess the improvement in your condition. Make sure to walk for 10 to 15 minutes after major meals and drink a glass of milk at night two hours before sleep, as it helps to reduce stomach acidity.

  3. Include a daily 1.25-mile walk to help resolve digestion-related issues.

Remember that the most significant impact on your reflux comes from your lifestyle. Medication can help, but if you want a permanent solution, then lifestyle modification is the best option. So, follow the steps mentioned above and schedule a follow-up call after 14 days, along with the 14-day treatment.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
