Why are my periods delayed, along with heavy flow?
Patient's Query
Hi doctor,
My periods were delayed by five days this month, and I also feel the flow is heavier, with significant pain on the third day. This is not how my cycles are usually. By the third day, generally, I would feel the blood flow would stop with no pain. Why is it unusual this month?
Kindly advise.
Hi,
Welcome to icliniq.com
I understand your concern. Normally, your cycle flow used to wind up fast, but this time period got delayed by five days, and now the bleeding is heavier, and cramps are still there even on day three. That change itself is important.
Periods can vary cycle to cycle, but when the pattern shifts suddenly, especially heavier and more painful, we usually consider causes like:
-
Hormonal imbalance (delayed ovulation, progesterone drop).
-
Sometimes thyroid issues.
-
Uterine structural causes like fibroid or polyp,
-
Just a one-off cycle variation.
I would suggest you get the following investigations done to fully understand the underlying issue.
-
Complete blood count- This is to check for anemia.
-
Thyroid profile.
-
Pelvic ultrasound- This is to look for fibroids or polyps and the endometrial thickness.
-
If cycles remain irregular, also consider serum prolactin and follicle-stimulating hormone or leutinizing hormone.
The differential diagnosis in your case can be:
-
Fibroid uterus
-
Endometrial polyp
-
Adenomyosis
-
Thyroid dysfunction
-
Dysfunctional uterine bleeding (anovulatory cycles).
In your case, it is mostly heavy menstrual bleeding due to hormonal imbalance.
I suggest the following treatment plan:
-
For now, if the flow is really heavy, that is, it is soaking pads every one to two hours, along with passing big clots, then you may need short-term hemostatic tablets (given after proper consultation).
-
Pain can be managed with non-steroidal anti-inflammatory drugs(NSAIDs) like Mefenamic acid. This must be taken only after checking for no gastric or kidney issues.
-
If this is just this cycle, it might settle on its own. But if the pattern repeats, you will need hormonal regulation (like combined pills or cyclical progesterone), but that needs in-person evaluation before starting. Consult your specialist doctor before starting any medication.
As preventive measures, you must
-
Track cycles properly.
-
Avoid stress, maintain regular sleep.
-
Keep hemoglobin up with an iron-rich diet.
-
Regular exercise helps regulate ovulation.
I would want you to update me if the bleeding continues beyond day seven or if you are soaking pads hourly. Also, please get an ultrasound and thyroid report, then we can decide further. If pain or heavy flow continues, consult a gynecologist to rule out a fibroid or polyp.
I hope this answers your query.
Thank you.
