Hello doctor,

I have been struggling with chronic hives for over eight weeks now, red, itchy welts that come and go daily, especially on my arms, legs, and neck. Antihistamines like Cetirizine and Levocetirizine help a bit, but they never fully go away, and I am losing sleep, scratching all night.

My IgE was 720, and nothing obvious is triggering this. I am really starting to feel helpless. When should stronger medicines like Omalizumab be considered? Could this be autoimmune?

I am also worried about the impact this constant inflammation is having on my body long-term. It is affecting my mood and work, and I just want to stop feeling like my own skin is attacking me.

It is reasonable to optimize antihistamines first by increasing the dose up to four times the standard amount under supervision. If symptoms remain uncontrolled after this step, adding Omalizumab, an anti-IgE monoclonal antibody, is the next recommended option and is highly effective and well tolerated in chronic urticaria.
