A 67-year-old immunocompromised patient who has undergone a lung transplant is experiencing severe pneumonia caused by human metapneumovirus (HMPV). This patient is 180 days post-transplant and is currently on a regimen of Tacrolimus and Mycophenolate Mofetil (MMF). The patient requires 3.95 gallons of OptiFlow and has an oxygen saturation level of 88 percent. A CT scan reveals diffuse ground-glass opacities (GGOs) with a crazy paving pattern. The bronchoalveolar lavage (BAL) shows a viral load exceeding 2 million copies. The patient is already receiving broad-spectrum antibiotics. A recent episode of rejection was managed with pulse steroid therapy. The transplant team is contemplating the use of ribavirin and evaluating the potential role of intravenous immunoglobulin (IVIG) while also expressing concerns regarding the reduction of immunosuppression.
