How to manage HMPV in infants with a heart condition?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My seven-month-old twins caught human metapneumovirus (HMPV) from daycare. One of them has Down syndrome and a heart defect. His oxygen levels keep fluctuating, and he seems more affected than his sister. He is already on heart medications. Can we give him additional medicine for HMPV? The other twin seems to be managing okay with just fever medicine. Should we take him to the cardiologist or wait? His last surgery was two months ago.

Kindly help!

Hello,

Welcome to icliniq.com.

I understand your concern.

There is no specific medication for HMPV, but supportive treatment can help. HMPV can cause upper respiratory tract infections, which may lead to oxygen fluctuations.

Monitoring and Immediate Care:

  1. Keep checking his oxygen levels. If it drops below 92 percent, take him to the hospital immediately.

  2. If his oxygen remains stable, you can follow these methods at home:

a. Use a humidifier.

b. Steam inhalation can help clear airways.

c. Use saline nasal drops.

d. Ensure rest and hydration. Give small sips of breast milk, diluted fresh juice, or chicken broth frequently. Avoid sugary drinks.

Cardiac Considerations:

HMPV does not directly cause heart problems, but infections can put stress on a weak heart. It is best to check with his cardiologist. Continue his heart medications as prescribed.

Preventing Further Infection:

  1. If there was an outbreak at daycare, avoid sending them back until all children recover. HMPV can still be contagious for up to two weeks after symptoms improve.
  2. Ensure the daycare has disinfected all surfaces and toys. At home, disinfect their belongings, wash their clothes in warm water, and clean toys with bleach-based cleaners.
  3. Try to separate the twins until both recover, as reinfection can delay healing.
  4. Since HMPV is contagious, use a mask while handling them and wash your hands frequently.

If either child's symptoms worsen, take them to the emergency room (ER) immediately.
