What could cause irregular periods?

Hi doctor,

I have had irregular periods for a few years and had regular cycles last four to five months. My last cycle was a little painful and I have been spotting light red or brown after a week of my last periods and it is continuous. I also have a certain discomfort around my lower right side near the uterus. I am really concerned.
