Which tests and medicines can help regulate my cycle?

Hello doctor,

I got my period 17 days earlier than expected. Could you please let me know the reason for this? We are currently planning to conceive, and my ovulation days are expected to last until next week.

I have read your query and can understand your concern.

According to the FIGO (International Federation of Gynecology and Obstetrics) classification of menstrual bleeding:

  1. Frequency:

Absent: No menstruation (amenorrhea).

Frequent: Less than 24 days between cycles.

Normal: 24 to 38 days between cycles.

Infrequent: More than 38 days between cycles.

2. Duration:

Normal: Up to eight days.

Prolonged: More than eight days.

3. Regularity:

Regular: Shortest to longest cycle variation is less than nine days.

Irregular: Shortest to longest variation is more than ten days.

4. Flow volume:

Heavy.

Normal.

Light.

A normal menstrual cycle, therefore, is characterized by:

  1. Frequency: 24 to 38 days.
  2. Duration: Up to eight days.
  3. Regularity: Less than nine days' variation.
  4. Flow: Normal.

I suggest the following investigations:

  1. Serum TSH (thyroid stimulating hormone), Free T4 (thyroxine), and Free T3 (triiodothyronine).
  2. Serum Prolactin.
  3. Serum FSH (follicle-stimulating hormone) and LH (luteinizing hormone) – to be done on the 2nd day of the menstrual cycle.
  4. Pelvic Ultrasound.

I suggest you following medications:

  • Tablet Dydrogesterone 10 mg (Progestin Hormone), twice daily for 21 days, starting from the 2nd day of the menstrual cycle. This will be a one month course.
  • Inomel-F Sachet (Myo-Inositol + Folic Acid + other micronutrients) (Nutritional supplement). One sachet mixed in one glass of water at night. For three months.
  • Tablet Folic Acid 5 mg (Vitamin supplement - B9). One tablet at night. For three months.

If you follow this treatment plan, it should help correct your irregular cycle and improve your chances of conceiving. Follow up for further queries.

Hello doctor,

This has happened only this month. My ovulation days were expected to last until a few days, and I got my period shortly afterward. My most fertile day was two days.

Do I need to start any medications now? Is there any issue that could affect my chances of conceiving?

I have read your query and can understand your concern.

You need to take the medication. First, we should work on correcting your menstrual cycle. Only after the cycle becomes regular can we accurately determine your ovulation period. If the cycle remains irregular, it is not possible to estimate the ovulation window correctly. Therefore, please start the medications as advised.

Once your cycle is stabilized, we can proceed with a transvaginal ultrasound (TVS) for follicular monitoring on day 12, starting from the first day of your next menstrual period. This will help us assess the maximum size of the ovum and determine the approximate timing of ovulation.
