Hello doctor,
I usually miss my period for more than three months and then again my period will start with spotting and later get heavy flow for another three months.

I understand your concern.
A menstrual (period) interval greater than 35 days is abnormal. For a period to occur, ovulation must have taken place, and pregnancy must not have occurred. Longer intervals like yours between periods suggest that most likely you are not ovulating. The cause varies; however, it is commonly PCOS (polycystic ovary syndrome). You would, however, need to do investigations to determine the cause so that appropriate medical management can be started.
In the interim, you should induce your period every six to eight weeks. This is essential to reduce the quantum of the period you get; as you wait for three months, the period will become very heavy and can distort your daily activities and cause anemia.
Investigations to be done:
Investigations to be done include a transvaginal ultrasound scan and hormonal profile such as FSH (follicle stimulating hormone), LH (luteinizing hormone), estrogens, testosterone, thyroid function test, prolactin, and progesterone. Additionally, fasting insulin levels should be tested.
Treatment plan:
I suggest you take a tablet Norethisterone acetate 10 mg every twelve hours for five days. Use it to induce your period after excluding pregnancy every six to eight weeks. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.
