Hello doctor,
I am a 29-year-old female, five feet three inches, 167 pounds, with a history of irritable bowel syndrome.
Over the past six months, I have experienced increased diarrhea with foul-smelling stools of varying colors (yellow, lime green, dark brown), abdominal and back pain, nausea, and fatigue, along with fluctuating weight.
My fecal occult blood test was negative, and liver ultrasound showed a 6.1-inch liver with a 0.55-inch echogenic nodule.
My current medications include Dextroamphetamine and Amphetamine extended-release 30 milligrams, Buspirone 10 milligrams, Lurasidone 20 milligrams, and Vybrid 10 milligrams.

These changes can occur when bile moves too quickly through the intestines or when fat digestion is impaired, which can point toward bile flow disturbances, pancreatic insufficiency, or malabsorption.
Since your fecal occult blood test was negative, gastrointestinal bleeding is unlikely, but further stool studies, such as fecal fat analysis, calprotectin, and stool elastase, can help evaluate inflammation or pancreatic function.
It is also important to check liver function tests, bilirubin, alkaline phosphatase, and possibly celiac antibodies to rule out other digestive or liver-related conditions.
