Can an IUD still be effective after years without periods?

Hello doctor,

I had a Kyleena IUD (intrauterine device) inserted four years ago. My period stopped completely after three months. However, three months ago, I started experiencing very light, pinkish spotting each month, lasting for two to three days. This month, I had bleeding more similar to a period (dark brown). After almost three years of having no bleeding at all, I want to determine whether this is normal and if my IUD is still effective despite the return of my periods.

My OB-GYN has retired, so I have been getting my pelvic exams and Pap smears done by my general practitioner. Additionally, I have lost almost 55 pounds over the past year (from 238 to 185 pounds) due to being on Mounjaro (Tirzepatide). I have type 2 diabetes and wonder if my weight loss could also be a contributing factor.
