Patient's Query
Hello doctor,
My wife is 52 and has been experiencing severe menopause symptoms for the past two years. She has hot flashes every hour during the day and night sweats so intense that she changes clothes three to four times a night. She has not had a period of 14 months. Her mood swings are affecting our marriage, she cries for no reason and gets angry over small things. A bone density scan showed osteopenia in her spine (T-score: -1.8), and her cholesterol has increased to 245 mg/dL. She has tried black cohosh and other herbal supplements, but nothing has helped. We are concerned about hormone replacement therapy because her mother was diagnosed with breast cancer at age 58. Is HRT safe in her case? What other options are available for managing menopause?
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I understand your concern.
I am concerned about your wife's severe menopause symptoms and the impact on your marriage. Hormone replacement therapy (HRT) can be effective for managing hot flashes, night sweats, and mood swings, but it does carry risks, especially with a family history of breast cancer. It is essential to discuss these risks with her healthcare provider to make an informed decision.
Other options for managing menopause symptoms include non-hormonal treatments such as SSRIs (selective serotonin reuptake inhibitors) or SNRIs (serotonin-norepinephrine reuptake inhibitors) for mood swings and hot flashes, and lifestyle changes like regular exercise, a healthy diet, and stress management techniques. For bone health, calcium and vitamin D supplements, along with weight-bearing exercises, can help manage osteopenia.
I hope this helps.
Kindly follow up if you have more concerns.
Thank you.
