Hello doctor,

I am 29 years old and have a 74-kilogram weight of five feet four height. I have been experiencing irregular menstrual cycles for almost a year now, although my cycles were fairly regular before. Over the past year, I got married and have been sexually active.

Do you have any other symptoms associated with your irregular menstrual cycle, such as:

  1. History of painful periods (dysmenorrhea)?

  2. Pain during intercourse (dyspareunia)?

  3. Bleeding between periods (intermenstrual bleeding)?

  4. Bleeding after intercourse (postcoital bleeding)?

  5. Any signs of weight gain, heat intolerance, palpitations, or cold intolerance?

  6. Any history of headaches, nausea, vomiting, or blurred vision?

  7. Weight gain, acne, or excessive hair growth (hirsutism)?

  8. Abdominal discomfort, difficulty breathing (dyspnea), or burning, urgent, or frequent urination?

To better evaluate the situation, I would recommend the following tests:

  1. Hormonal tests:

    1. Serum FT4 (free thyroxine) and TSH (thyroid stimulating hormone) (to check thyroid function).

    2. Serum FSH (follicular stimulating hormone) and LH (luteinizing hormone) on the 2nd day of your menstrual cycle.

    3. Serum prolactin level.

  2. Ultrasound of the pelvis to check for any abnormalities.

In the meantime, I would suggest starting the following medications for the next three months:

  1. Tablet YAZ (contains Ethinylestradiol 0.02 milligrams and Drospirenone 3 milligrams):

    • Take for 21 days, starting from the 2nd day of your cycle, followed by a seven-day break. Repeat this routine for three months.

  2. Inomel-F sachet:

    • Take one sachet at night for three months.
