Hi doctor,

I have been having spotting between my periods for the past seven months. Spotting always appears between my periods, and it lasts for a day or two. It always happens around the same time. I believe it is not from ovulation. I have had normal periods overall. In the past, I have faced the same issue. Please help.

I understand your concern.

Spotting in between periods can be caused by quite a number of things ranging from normal hormonal fluctuation to structural problems in the uterus. Sometimes, there is spotting a few days after the period. It is usually pinkish or brownish in color. This spotting is usually a remnant of the period. Also, women with structural abnormalities like fibroids or polyps sometimes have spotting between their periods. This structural abnormality can be confirmed with sonography, and appropriate management should be instituted. Occasionally, if an individual commences contraceptives, there could be spotting in between periods, but this should not last longer than five months. Or if there was a preceding trauma (there would be a history of pain during sex). So, you would need to do at least a sonogram (ultrasound scan) to rule out structural anomalies and an STI (sexually transmitted infections) screen to rule out cervicitis, which could also cause intermenstrual spotting (there could be discharge and or itching). If these tests are normal and you have not recently started any medication, then it is most likely hormonal fluctuation that is causing the spotting between periods.

Thank you for your answer.

A year ago, I did a scan, and it came back normal, apart from the fact that one of my ovaries is polycystic. I will follow up with my clinician, although it seems like this is because of some hormonal imbalance, as it always happens during the same day of my cycle (12th day).

I understand your query and concern.

If it occurs on the 12th day, it is a hormonal imbalance. Just before ovulation, there is a drop in estrogen in some women, prompting spotting. This should not be a source of concern, and it normalizes after some time. However, you could still follow up with your clinician for the polycystic ovaries.
