Hello doctor,

I am a 21-year-old girl who has been dealing with OCD for the past four years. While I am undergoing treatment, I have a specific doubt and concern that I would like to address.

Recently, I experienced a breakup in my seven-year relationship, and it was due to infidelity. We had a sexual relationship last year, and what makes it more complex is that he is my cousin. Due to my OCD, I constantly obsess over the idea that I am a bad person for engaging in a sexual relationship with my cousin and for having sex before marriage.

I have tried to rationalize it to myself countless times, explaining that cousin relationships and consensual sex between adults are not inherently wrong. However, my mind remains fixated on these thoughts, making me feel guilty.

I am seeking your assistance in overcoming this persistent guilt and reassuring me that I have not done anything wrong. Kindly help.

Thank you for taking help from a psychiatrist. I can understand your situation. You have Obsessive-Compulsive Disorder. There is non-pharmacological management for it, you should try to follow the below guidelines to deal with it that will help you.

  1. You have to move on in your life from your cousin.

  2. Stop thinking about him.

  3. Accept your past, that you cannot change.

  4. You should do exercises daily for 30 to 45 minutes like at least going for a walk.

  5. You should do meditation for 20 minutes daily, just sit in one place in a comfortable position and try to concentrate on your normal breathing, do not stop or force yourself to stop your thoughts, just let it be. It will reduce your thoughts.

  6. You should eat healthy home-cooked foods, and avoid junk foods.

  7. You must drink enough water.

  8. You should spend quality time with friends and family daily or do video calls if you are living away from your family.

  9. You should spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills or language or musical instruments.

  10. You should get enough seven to eight hours of sleep daily.

  11. You must avoid tobacco, alcohol, or any substance abuse.

  12. Live your life, do not just spend it.

  13. Be bold and confident.

You have not mentioned your medication details. You need to start capsule Fluoxetine 20 mg daily in the morning after breakfast. If you need a prescription then you need to do a video consultation.
