What causes severe menstrual pain in a 25-year-old?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

When I was 16 years old, I experienced sudden abdominal pain. When I went for an ultrasound, it revealed that there was a cyst in my uterus that had ruptured, and the pain was due to that rupture. Suddenly, my doctor planned a surgery and removed the ruptured cyst.

Now I am 25 years old, and for the past year I have been experiencing severe menstrual pain that I cannot bear, even painkillers are not effective for long.

I am experiencing the following symptoms:

  1. Severe period pain.

  2. Mood swings.

  3. Backache with period pain.

  4. Periods are arriving ten days to one week earlier than expected.

  5. Faster and increased hair growth on my face and body.

  6. Slight weight gain.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

From your history and your detailed description, I believe that you have a condition called polycystic ovarian syndrome or endometriosis. Endometriosis is one of the most common causes of severe pain during periods. In your case, mood swings and facial hair are explained by polycystic ovarian syndrome. Hormonal imbalance, androgen and testosterone levels may be high compared to estrogen levels. You may have a recurrence of ovarian cysts.

I suggest you follow the below mentioned instructions below:

  1. See a gynaecologist for further evaluation.

  2. Keep track of your cycles.

  3. Perform a urine pregnancy test if you are sexually active.

Treatment differs based on the diagnosis and will be explained once we obtain your test results and imaging, such as an ultrasound or MRI (magnetic resonance imaging).

Kindly follow up if you have more doubts.

I hope this helps.

Thank you.
