Hello doctor,

I have a very itchy skin for the past eight months. I have small red spots and large hive-like spots that started on my forearms and are now appearing on my hands, torso, and legs. I underwent treatment for scabies and folliculitis conditions as suggested by my GP. I was prescribed tablet Fexofenadine for itching and an antibiotic Flucloxacillin for folliculitis on my calf. I have not taken any lab tests as of now. The itching and rashes are progressively getting worse and are very visible. It is all over my hands and forearms currently and it is not so bad on the other body parts. I have no history of allergies, but I had asthma when I was a child. There is a history of eczema in my family. Please help.

I went through your query and concern, and I examined your pictures (attachments removed to protect the patient's identity). It seems that you have developed papular urticaria. The causes of papular urticaria are as follows:

  1. Infections (bacterial, viral, fungal, or parasitic).
  2. Drugs (painkillers or antibiotics etc.).
  3. Certain foods and food additives or preservatives.

You have to rule out the cause to obtain a permanent cure. I suggest you take Levocetirizine and Montelukast combination tablets once daily for around one month. Also, take deworming tablet like Albendazole 400 mg (single dose-once only). A topical soothing lotion like zinc oxide and 0.5 % iron oxide (Calamine) can be applied locally twice daily for five to seven days.
