Why am I having penile discharge and pain after sex?

Hello doctor,

I had sexual intercourse with a girl, and two days later, I started facing some problems. There is a grey-white fluid discharge from my penis continuously. I also experience pain and irritation in the penis while passing urine.

Based on the symptoms, grey-white discharge from the penis along with pain and irritation while passing urine, appearing two days after sexual contact, this may be due to a local infection. The infection may be bacterial or fungal in nature. If the sexual contact was unprotected, sexually transmitted infections must also be ruled out.

If there was unprotected sexual contact, the following tests are advised:

  • TPHA (treponema pallidum hemagglutination assay).

  • HSV type 1 and type 2 NAAT or RT-PCR (herpes simplex virus nucleic acid amplification test or real-time polymerase chain reaction).

  • Do not do herpes simplex virus Immunoglobulin G, as it is not useful for diagnosis.

  • Urine PCR (polymerase chain reaction) for Chlamydia trachomatis and Neisseria gonorrhoeae.

  • HIV (human Immunodeficiency Virus) 4th-generation antigen or antibody test.

  • HBsAg (hepatitis B surface antigen).

  • Urine routine examination with culture and sensitivity.

  • CBC (complete blood count) and ESR (erythrocyte sedimentation rate).

These investigations help identify or rule out common causes of penile discharge, irritation, and pain following sexual exposure.

If there was no unprotected sexual contact:

  • If the discharge appears bacterial (sticky discharge, foul smell, painful urination), apply Mupirocin ointment locally twice daily.

  • Take Doxycycline 100 milligrams twice daily for five days, after food.

  • If there is significant itching or redness, apply Clotrimazole with Beclomethasone ointment locally twice daily.

  • Take Cetirizine 10 milligrams once daily at night for itching.

General genital hygiene advice:

  • Wash the genital area twice daily, in the morning and at night.

  • Use normal room-temperature water.

  • Avoid hot or very cold water.

  • Do not use harsh or scented soaps.

  • Keep the area dry and avoid tight underwear or clothing.

If possible, a clear photograph of the affected area (without the face or any identifying features) may help in providing more accurate guidance.
