Hello doctor,
I have a tear on my perineum that appeared today, and I am unsure why. I have been dealing with a hemorrhoid for the last few days and just started using Prep H cream.
I just started using the hemorrhoid cream two days ago. This was not here until today. I am not sure if I am having a reaction to the creams. I do not know if that has caused irritation, but I am terrified, as most searches state that HSV could be a cause.
I do not think that is what it is, but I am scared.
Please help.

Herpes usually causes significant burning pain, clusters of blisters, and a more progressive rash.
To better assess the situation, it would be very helpful to see a photo of the area and also clarify whether there is significant pain or burning, any blisters, discomfort during urination, or any previous similar episodes.
At this stage, if it appears to be a small fissure without blisters, you can temporarily stop the irritating cream and use a soothing healing ointment such as Bepanthen (Dexpanthenol) two to three times daily for five to seven days, avoid friction, and keep the area clean and dry.
If symptoms worsen, blisters appear, or there is no improvement within a few days, you should seek medical evaluation.
