Hi doctor,

I think I might have premenstrual dysphoric disorder (PMDD) or severe PMS.

About one to two weeks before my period, I experience intense mood swings, crying spells, anger, body soreness, severe headaches, brain fog, and a lack of motivation. These symptoms usually last for a week or two before my period starts. My periods are also irregular and sometimes occur every two months.

Please help.

I understand your concern.

Your symptoms are suggestive of PMDD (premenstrual dysphoric disorder) if they have been present for more than two cycles, begin to fade when menstruation starts, and completely resolve afterward.

If the symptoms persist throughout the month but become worse before your menstrual cycle, it could be due to an underlying anxiety disorder that worsens with hormonal changes.

PMDD is a manageable condition, and the best option is to start Sertraline 50 mg in the morning. It can be taken either continuously or intermittently for 14 days before your period; both approaches are effective.

You can also take Ibuprofen to help with pain-related symptoms.

If antidepressants do not provide relief, hormonal treatments can be considered, although SSRIs (selective serotonin reuptake inhibitors) like Sertraline are usually effective. You may also take Calcium and Vitamin B6, as they have shown some supportive evidence; however, the mainstay of treatment remains an SSRI.

Sertraline can cause mild gastrointestinal side effects such as bloating and nausea, but these usually subside after two to three days.

Irregular menstrual cycles can have multiple causes, including thyroid dysfunction, hormonal imbalance, or elevated prolactin levels. Please consult a gynecologist for a detailed assessment.

Recommended investigations include:

  • Thyroid profile.

  • Ferritin.

  • Vitamin B12.

  • Vitamin D.

  • Prolactin levels.
